Since Dr. Klingensmith owns the practice, she has the luxury of determining how much time to spend with each patient. On a first visit she will get to know you and your lifestyle and medical history.

For the utmost convenience, you can fill out the forms online before your appointment. If you have not done so, please come in 15 minutes before your appointment.

What You'll Need to Get Ready For Your First Visit

If you haven’t already filled out the online forms, please come at least 15 minutes early to fill them out at our office.

Bring your ID, your insurance card and any previous diagnostic testing or X-rays.
